A 504 Plan is a formal agreement between a school and a student with a disability that guarantees the student a free and appropriate public education (FAPE). 504 Plans are designed to provide students with disabilities with the accommodations and modifications they need to succeed in school.

Any student with a physical or mental impairment that substantially limits one or more major life activities is eligible for a 504 Plan. Major life activities include walking, seeing, hearing, speaking, breathing, learning, caring for oneself, performing manual tasks, standing, sitting, and reaching.

To request a 504 Plan for your child, you should contact your child's school principal or special education director. The school will then convene a team to evaluate your child and determine if they are eligible for a 504 Plan.
